City of Chicago Residents

City of Chicago-operated mass vaccination sites located across the city offer appointments to Chicago residents. Appointments are preferred and can be scheduled via a centralized booking site. Walk-ins are accepted. Proof of residency is required. Learn more.

Illinois residents ages 12+, including Chicago residents, can seek appointments:
